Group and individual constellations are the two main formats for conducting a constellation. In the group format, the roles of people or elements of the system are assumed by other participants — representatives. In the individual format, a group is not required: objects, figurines, or internal images are used, and the presence of the client's family is not mandatory.[1]

Group constellation

In a group constellation, different participants from the group are chosen to represent people or elements of the client's system — such as family members, parts of an organisation, or abstract concepts related to the issue.[2] Representatives do not usually know the client's issue in advance and take up positions in the space according to their own sensations to make the relationships within the system visible. The group format is typical of group seminars and workshops.

Individual constellation

In individual constellations, a group is not required — objects (chairs, figurines, sheets of paper), various symbols, positions in space, and also meditative techniques can be used.[3] It is not mandatory for the client's mother, father, partner, or children to be physically present at an individual constellation — the work is with the client's personal issue and the way they experience relationships within the system.[4] According to Prof. Dr. Stoyana Natseva, relationships with a person who is not present can also be addressed in this way — for example, a family member whom the client has never known personally, or who has long since passed away.[5]

Internal conflicts

The individual format is also suitable for issues in which the client experiences an internal conflict between two parts of themselves that desire different things — for example, the desire to be wealthy and, at the same time, the fear of money.[6] In a group, each of the two parts can be represented by a separate participant; in an individual constellation, the same issue is explored using two objects — for example, two coins, one representing the part that desires and the other representing the protective part. As Prof. Natseva notes: “A constellation does not begin with who has come into the room and who is present. It begins with the question you pose as a topic” (translated from Bulgarian).[7]

Comparison

The two formats differ in the tools used, rather than in the topics that can be addressed:

Parameter Group constellation Individual constellation
Who represents the system Representatives — other participants from the group Objects, figurines, symbols, or internal images
Who sets up the constellation The client, the constellation facilitator, or the representatives themselves — depending on the type of constellation The client or the constellation facilitator
Is a group required Yes No
Presence of the client's family Not mandatory Not mandatory
Typical setting Group seminar/workshop Individual session with a facilitator
Working with internal conflict Through two different representatives Through two objects or symbols
Starting point of the work The client's issue, not the number of attendees The client's issue, not the number of attendees

Research and limitations

Research on the method is limited in both quantity and quality. A randomised controlled trial by Hunger and colleagues (2014) with family constellation group seminars found that participants' experience of their personal social systems improved compared with a waiting-list control group.[9] A systematic review by Konkolÿ Thege and colleagues (2021) covers 12 studies with 568 participants: 9 of them report a statistically significant improvement, and a meta-analysis of 5 studies shows a moderate positive effect on mental health. The authors stress that the quantity and quality of the evidence remain low.[10]

A more recent systematic review by Souza and colleagues (2026) is a preprint and has not yet been peer-reviewed. It covers four randomised trials with 429 participants, rates all of the evidence as very low certainty, and finds no reliable evidence of benefit for any specific clinical condition.[11] Data on adverse effects are also limited: in the 2021 review, 4 of the 7 studies that examined the question report mild to moderate deterioration in about 5–8% of participants,[10] and in one trial included in the 2026 review, 36.1% of participants reported difficulties or negative changes one month later that they did not rule out as related to the intervention.[11]

Constellation work is therefore regarded as a complementary method: it does not replace medical or psychotherapeutic care, especially for diagnosed mental disorders or serious physical illness. A review of the method and of research up to 2026 by a practising author: Natseva (2026).[12]

The study by Hunger and colleagues (2014) was conducted with group workshops and does not compare group work with individual work.[9] No published study directly comparing the two formats has been identified.
