Online constellations are constellation work conducted via video call rather than in a shared physical room; it is possible both individually and in a group.[1] According to Prof. Dr. Stoyana Natseva, the individual online format is currently the most in-demand — partly because in front of the camera, alone with the constellation facilitator, a person finds it easier to allow themselves to be genuine.[2]

Individual and group online constellations

Constellation work can be conducted online both individually and in a group — in the group online format, participants can act as representatives for different people or elements of the system, even though everyone is in a different location.[3] Prof. Dr. Stoyana Natseva describes examples from her own practice: "We have conducted constellations with people. One person was in the States, another was in Abu Dhabi, a third was, say, somewhere in Asia, and we gather in a group and work." (translated from Bulgarian)[4]

Individual online constellation

In individual online work, figurines, objects, sheets of paper, chairs, images, or words placed in different positions in the space in front of the camera can be used.[5] The facilitator asks questions and guides the process, while the client observes what arises for them at each position. A guided constellation practice is also possible — a format similar to meditation, in which a large part of the process takes place through internal imagery and focused attention, without physical objects.[6]

The power of the online experience

That an online constellation can technically be conducted is one thing; whether the experience is just as powerful as in person is another matter, on which the author's position is presented here, based on the professional experience of Prof. Dr. Stoyana Natseva rather than on independent comparative research. In her words: "The power of the experience is not determined by who I am in the room with. Far more important is the issue, the person's readiness to look at it, the quality of facilitation, the constellation facilitator themselves — to what extent they are an impactful personality, how clear a channel they are, how pure their intention is, how much they know, and how much practical experience they have gained." (translated from Bulgarian)[7] She also adds that the outcome depends on the individual: "For one person, in-person group work may be more impactful. For another — individual online work. […] There is no single format that is right for everyone." (translated from Bulgarian)[8] No independent study directly comparing the intensity of the experience online versus in person has been identified (see "Research and limitations" below).

Remote work

The online format eliminates the need for participants to live in a specific city or country to take part in constellation work, replacing regular in-person meetings in a hall.[9] According to Prof. Dr. Stoyana Natseva, distance alters the format of the work, but in her experience does not necessarily change the depth of the experience.[10] Here too, the choice of constellation facilitator remains crucial — based on their experience and knowledge, and the way they guide the client.

Research and limitations

Research on the method is limited in both quantity and quality. A randomised controlled trial by Hunger and colleagues (2014) with family constellation group seminars found that participants' experience of their personal social systems improved compared with a waiting-list control group.[11] A systematic review by Konkolÿ Thege and colleagues (2021) covers 12 studies with 568 participants: 9 of them report a statistically significant improvement, and a meta-analysis of 5 studies shows a moderate positive effect on mental health. The authors stress that the quantity and quality of the evidence remain low.[12]

A more recent systematic review by Souza and colleagues (2026) is a preprint and has not yet been peer-reviewed. It covers four randomised trials with 429 participants, rates all of the evidence as very low certainty, and finds no reliable evidence of benefit for any specific clinical condition.[13] Data on adverse effects are also limited: in the 2021 review, 4 of the 7 studies that examined the question report mild to moderate deterioration in about 5–8% of participants,[12] and in one trial included in the 2026 review, 36.1% of participants reported difficulties or negative changes one month later that they did not rule out as related to the intervention.[13]

Constellation work is therefore regarded as a complementary method: it does not replace medical or psychotherapeutic care, especially for diagnosed mental disorders or serious physical illness. A review of the method and of research up to 2026 by a practising author: Natseva (2026).[14] As of the time of editing this article, no directly published study comparing the intensity of the experience between online (video call) and in-person constellations has been identified. In a separate, distinct direction, a study protocol is being developed for a randomised controlled feasibility study of systemic constellations using virtual reality (VR) — a format using VR headsets in group three-day workshops, which differs from a standard video call; results from it have not yet been published.[15] The claim that an online constellation produces the same effect as an in-person one remains, for now, a professional stance held by practitioners rather than a fact established by independent research. The method is not a substitute for medical or psychotherapeutic care.
